The SDM application for admission has several components:
Application form: Please complete the online application form.
Three letters of recommendation. Evaluations should be completed by individuals who can comment with some certainty about an applicant's intellectual and professional achievements and leadership potential. If possible, one evaluation should be completed by an academic source for applicants who attended a university within the last five years. Download this evaluation form and send it to your evaluators.

GRE or GMAT scores: Applicants must submit current GRE or GMAT scores. Test scores should be sent to the SDM program office. The SDM program code is X5X-QS-67. The average GRE and GMAT scores for the Cohort for 2008 were:

Transcripts: One copy of an official transcript is required from each post-secondary institution attended (for degree programs or single courses). Transcripts from international (non-US) institutions must be in English and certified.
TOEFL: Non-native English speakers who have not obtained a post-secondary degree at an English language institution are required to submit scores from the Test of English as a Foreign Language (TOEFL). Test scores should be sent to the SDM program office. The SDM program code is 3536.
Application fee: An application fee of $70, in the form of a check made out to MIT, must accompany each application.
